Submission Form

1. Names: Please list all participants first and last names that should be credited for your video.
2. Point-of-Contact and Phone Number: For your video submission, please list the name of the individual who is your video point-of-contact and a single phone number for him/her.
3. Point-of-Contact Address: Please list a single address to be used for an award check.
4. Company Name (if applicable).
5. Company Address, Street, City, State, Zip (if applicable).
6. Comment about the Video Format: The video producer should comment on the video. This can include the video format, any specific details, story line or special effects utilized, and any particular inspiration as to why you chose to submit a video. Please Note: Kings of Crownsville may choose to publish and make public the video producer's comments.
7. Include a copy of your verification email of the WeTransfer.com receipt.
8. For every submission, Kings of Crownsville will send a verification email of receipt of your video. If you do not hear from us within 4 working days, please send an email to: videocontest@kingsofcrownsville.com
   
9. Important: Send a completed Submission Form as a scanned attachment within an email to: videocontest@kingsofcrownsville.com
